I do hereby give my son _______________________________________ permission to attend a weekend camping trip with the members of Boy Scout Troop to Camp Lewis Scout Camp in Rockaway, NJ. The cost of the trip will be $20. It will cover all the meals for the weekend. We will be departing from the St. Anthony’s parking lot on Friday, March 19th 2010 @6 PM (Please be on time!) We will return from the trip at or about 1:30 PM on Sunday, March 21st 2010 to St. Anthony’s parking lot.
I do hereby, give Scoutmaster David Ovejero full responsibility as my son’s guardian for the weekend. If anything should happen, my son will be taken to a duly authorized physician under the leadership of the Scoutmaster. Parent will be notified if an accident may occur during the weekend camping trip. During my absence, Mr. Ovejero is acting guardian of my son.
Note: Permission slip must be signed and returned in order to attend camping trip. Please pack accordingly to the weather for the weekend.
